Important Note
For feline diabetes studies, the questionnaire should be completed by the pet owner. Please direct owners to the "Pet Owner" section to complete the online questionnaire and obtain their unique participant code.
Instructions for Veterinarians
- Direct the pet owner to complete the online questionnaire (Pet Owner section)
- Owner will receive a unique participant code upon completion
- Download the laboratory submission form (PDF or Word)
- Fill in the laboratory form with patient details and the owner's unique code
- Collect samples according to the protocol
- Submit samples with the completed form to the RVC laboratory
