Summary Instructions
- Complete the online questionnaire to get your unique participant code
- Write down your participant code (you'll receive it after submitting the questionnaire)
- Download the laboratory submission form (PDF or Word)
- Fill in your unique participant code on the laboratory form
- Give the completed form to your veterinarian
- Your veterinarian will collect samples and submit them to the RVC laboratory
